ADMISSIONS
Admission to Penn-Mar’s programs begins with an application, which can be filled out by applicants themselves or by qualified family members or guardians. Along with an application, Penn-Mar asks for the most recent psychological evaluation, social history, physical exam, dental exam, psychiatric evaluation when indicated and all other heath screenings indicated by the primary care physician or specialist. All medical exams and health screenings must take place within one year of admission, unless otherwise requested.
The quality assurance team reviews each request and the supporting documentation to determine the appropriate placement. If admission is granted, the applicant is required to have had a physical examination within the previous six months, must be free from communicable diseases and must have received, or be in the process of receiving, the Hepatitis B vaccine. These stipulations are in addition to all other requirements of the state in which services are funded.
Placement in any setting is based on needs, compatibility and other pertinent factors relative to each individual. As individuals and situations change, an individual may have to change his or her work or home location. Each individual’s team members will discuss recommended changes before they occur and seek to reach an agreement before making any moves. However, we recognize that some changes may occur without the agreement of all team members.
Services will not be provided without funding, which may come through state funds or private sources.
